Y257H, A116T, D116E, others (less frequent / isolate specific)

Individual reports show variable effects (from small increases up to moderate/high increases depending on background); many of these are reported as contributing mutations in resistant isolates rather than sole drivers. Typical reported increases: 2–16-fold for fluconazole in various reports.

Generally variable and often smaller for voriconazole unless combined with other mutations (2–8-fold typical).

These substitutions are reported across geographically diverse clinical collections (HIV-associated fungemia, hospital outbreaks, surveillance panels). They often act additively with efflux or expression changes.
